Our experienced team of care management specialists and friendly companions are there to ensure the highest quality of care is consistently provided. Our caregivers schedules are flexible and are available to provide care whenever and wherever it’s needed, while our clients receive the assistance they need to continue to enjoy their regular activities.

Nursing services are provided by our Nurses working closely with Primary Care Physicians or Hospice Medical Directors to create and implement a care plan that is unique and tailored to each individual and their needs.

Hospice Care is for patients and their families facing terminal illness and for whom curative care is no longer beneficial. Our emphasis is on quality of life, on comfort, and compassionate care. Medical Social Workers are available to offer solutions to financial, social and emotional concerns of the patient and family. Changes precipitated by a health care crisis can be devastating for the family; the social workers at Journey Hospice provide counseling and coordinate support the most complex illnesses and treatment plans that often encompass the entire healthcare and social gamut.

Our philosophy of bereavement support encourages expression of grief through listening, normalizing and validating an individual’s unique experience. Many of our volunteers have personally benefited from hospice care during a loved one’s serious illness, and they want to help families facing the same difficult time. Others are compassionate individuals who simply want to give of themselves in a very special way.

Our hospice volunteers often work directly with patients and families, providing compassion, companionship, and emotional support. In many cases, the hospice volunteer becomes a true and trusted friend to a patient and family.

Volunteers donate their time and efforts in a wide variety of ways, such as reading to, talking with, or writing a letter for a patient. They may prepare a meal, provide transportation, run an errand, or give a family member a little time off from the role of primary caregiver.
