Primary function is to provide personal healthcare and related services to the patient in their place of residence; to assist in proving a safe and clean environment, work cooperatively with patient and family and share observations and problems with the supervisor.

Qualifications:

  • Education: High School Diploma preferred
  • Has successfully completely one of the following:
    • Training program/competency evaluation as specified in §418.76(b)(c) of the Medicare regulations; 16 additional hours of hospice training over the 75 hr. HHA training program requirement
    • Competency evaluation program that meets §418.76(c);
    • Nurse aide training/competency evaluation program approved by the State and listed in good standing on the state nurse aide registry;
  • Experience: At least one (1) year experience, preferred, as a Hospice Aide or Nursing Assistant in a hospital, nursing home, home health agency, hospice, or long- term care facility or completion of Certified Nursing Assistant or Certified Home Health Aide Program
  • Skills: Must be able to read and write in English and follow written and verbal instructions in English effectively
  • S/he demonstrates interest in the welfare of ill and elderly
  • Must pass Aide Competency Evaluation Skills Checklist that meets §418.76(c) and written Skills test

Primary function is to provide personal assistance services to the patient under the interdisciplinary team plan of care, in their place of residence; to assist in providing a safe and clean environment, work cooperatively with patient and family and share observations and problems with the supervisor.

Qualifications:

  • Education: High School Diploma preferred
  • Licensure: Must have current Driver’s license
  • Experience: One (1) year experience in providing services to individuals needing homemaker/companion assistance is preferred
  • Skills: Must be able to read and write in English and follow written and verbal instructions in English effectively
  • S/he demonstrates interest in the welfare of the ill and elderly
  • S/he must be competent to perform tasks assigned by supervisor

Primary function is to act as liaison between (agency name) and the community regarding community and customer needs. Responsibilities include development and promotion of hospice programs and services while maintaining the standards of practice consistent with quality health care and maximizing human, financial and equipment resources.

Qualifications:

  • Education: Graduate of an accredited college/university is preferred
  • Experience: Experience in the health care field preferred
  • Skills: Ability to establish and maintain effective working relationships with all segments of the staff, the lay and professional public, the Board of Directors and Medical Director

The Licensed Practical/Vocational Nurse is responsible for providing direct patient care under the supervision of a registered nurse. Responsibilities include following the plan of care, providing treatments, and working collaboratively with the members of the team to help meet positive patient care outcomes.

Qualifications:

  • Graduate of an accredited practical nurse or vocational nursing program
  • Three years nursing experience. Community health/hospice or medical/surgical experience is preferred
  • Currently licensed as an LPN/LVN in TX
  • Complies with accepted professional standards and practice
  • Demonstrates good verbal and written communication, and organization skills
  • Must be a licensed driver with an automobile that is insured in accordance with TX and/or agency requirements and is in good working order
  • Possesses and maintains current CPR certification
  • Understands philosophy of hospice concept. Also understands needs of the terminally ill

A registered professional who provides coordinated care to patients of all age groups. S/he plans, implements and evaluates patient care plans to restore or maintain patient well-being. S/he evaluates occupational roles and occupational performance of patients with physical, psychosocial and/or cognitive impairments. Provides intervention to deficits and elicits responses. Collaborates with interdisciplinary team.

Qualifications:

  • Education: Bachelor of Science in Occupational Therapy
  • Licensure: Current State License as an Occupational Therapist, current state driver’s license. Experience: Two years’ experience as an occupational therapist in an acute care or rehabilitation setting
  • Skills: Therapy skills as defined as generally accepted standards of practice. Good interpersonal skills
  • Proof of hepatitis profile
  • Knowledge of durable medical equipment

A registered, currently licensed professional who provides coordinated care to patients of all age groups. S/he plans, implements and evaluates patient care plans to restore or maintain patient well-being. Plans, organizes and conducts physical therapy treatment programs based on the medical referral and their evaluation. S/he collaborates with the interdisciplinary team.

Qualifications:

  • Education: Bachelor of Science in Physical Therapy from an American Physical Therapy Association approved program
  • Licensure: Current (State) License as a Physical Therapist, current (State) Driver’s License
  • Experience: Two years’ experience as a Physical Therapist in an acute care or rehabilitation setting
  • Skills: Therapy skills as defined as generally accepted standards of practice
  • Good interpersonal skills
  • Proof of hepatitis profile
  • Knowledge of durable medical equipment

SLPs contribute to the overall quality of life of patients nearing end of life. The goal of intervention at this point is not rehabilitative, but facilitative. A Speech Language Pathologist is a registered professional who provides therapeutic techniques for the facilitative interventions of patients with speech, language, hearing, and oral motor, swallowing and cognitive disorders at or near the end of life. S/he collaborates with Interdisciplinary team.

Qualifications:

  • Education: Master’s degree in Speech-Language Pathology from an accredited college or university
  • Licensure: Current State License in Speech-Language Pathology, Certificate of Clinical competence from American Speech-Language Hearing Associates
  • S/he must have current State Driver’s License
  • Experience: Two years’ experience in an acute care or rehabilitation setting, home health or hospice preferred
  • Skills: Therapy skills as defined as generally accepted standards of practice
  • Good interpersonal skills and proof of Hepatitis profile

Under the supervision of the Director of Hospice Services, provides technical, educational and consultative services in the field of nutrition.

Qualifications:

  • Graduate from a college or university with a Baccalaureate degree in dietetics or foods and nutrition, plus an ADA-approved internship required
  • Current registration with the American Dietetic Association
  • Possess a valid (State) drivers’ license
  • A minimum of one (1) year as a registered dietitian in a health agency or hospital
  • Above average interpersonal and verbal communication skills
  • Ability to establish goals and monitor progress against goals
  • Ability to make quick, sound decisions, utilizing critical thinking skills
  • Possess strong public relations skills
  • Ability to handle multiple assignments at the same time with limited supervision
  • Physical Requirements:
    • Body Positions: Sitting, standing and driving
    • Body Movements: Arm and hand dexterity
    • Body Senses: Must have command of close and distant sight, color perception and hearing
  • Strength: Ability to lift and move up to 20 pounds

The registered nurse plans, organizes and directs hospice care and is experienced in nursing, with emphasis on community health education/experience. The professional nurse builds from the resources of the community to plan and direct services to meet the needs of individual and families within their homes and communities.

Qualifications:

  • Graduate of an accredited school of nursing, with one (1) to two (2) years of recent acute care experience in an institutional setting, and one (1) to two (2) years of recent experience in home care
  • Current licensure in State, CPR certification and valid driver's license
  • Bachelor's degree, with one (1) year of hospice care experience preferred
  • Responsible for supervising hospice aides
  • Excellent observation, verbal and written communication skills, problem solving skills, basic math skills; nursing skills per competency checklist
  • Prolonged or considerable walking or standing
  • Must be able to lift or position and/or transfer patients
  • Must be able to lift supplies and equipment
  • Considerable reaching, stooping bending, kneeling and/or crouching
  • Visual acuity and hearing to perform required nursing skills
